C8Prov has exclusive services that health insurance plans do not cover for C8Prov's valued clients.
This plan includes frequent one-on-one visits with personalized care provided by Nurse Practitioner Linh McClure, DNP, APRN, AGNP-C, a primary care general nurse practitioner with direct access.
Frequent visits include in-home visits (only in central and northeast Florida), audiovisual telehealth, and in-office clinical visits, scheduled by appointment.
Integrated and individualized healthcare and primary preventive care services.
Acute and chronic disease prevention and management.
Weight loss, individual ideal weight management, & healthy lifestyle coaching services are included.
Referrals to specialist physicians when appropriate, such as cardiologists, pulmonologists, rheumatologists, endocrinologists, gastroenterologists, and psychiatrists.
Recommendations are also made to other specialist allied health professionals (e.g., physical therapists, psychologists, mental health counselors, registered dietitians or nutritionists, and chiropractors), as necessary.
